Fixes from the docs review

The **Error workflow** setting runs only for a failed top-level production run. A **Return Response** answer carries the run’s headers. A blank template name is a `400`. **Run again** on a test run reruns the version that ran.

## Going back

To go back, run `./deploy.sh v2026.10.30`. There is no migration, so no flag is needed.
